Service Provider Terms & Conditions

General Terms

  • No Service Provider may work on site at any of the satellite Heart Place Hospital locations or provide a service under the Heart Place Hospital umbrella, work with, consult, or provide advice/treatment to HPH visitors without first agreeing to and signing these Practitioner T&Cs 

  • Professional standards: Service providers are expected to adhere to professional codes of ethics and conduct. These standards govern behaviour, clinical practice, confidentiality, and interactions with visitors

  • Service providers are expected to obtain informed consent from visitors for treatment and services, as well as maintaining visitor confidentiality and privacy in accordance with standard laws (Privacy Act 2020 and the Privacy Principles)and regulations. 

  • Fees and billing: Prior agreement on service provider's fee structure, payment methods, cancellation or rescheduling policies, and any financial responsibilities of the visitor before commencing services.  

  • Scope of practice: Each Service Provider must specify the specific services or treatments offered by the service provider within their area of expertise and competence

  • Record-keeping: Service providers are required to keep records of each visit.

  • Cancellation policy: see hire agreement for workshops or satellite clinics 

  • Professional liability: Service Providers are advised to have their own professional liability insurance.  

  • Reviews and reporting. All practitioners of HPH are required to produce and annual review by the 1st May each year

  • Marketing: approved service providers will feature on Heart Place Hospital website and menu for all visitors.  See Hire Agreement for more information

 

Space Hire Agreement 

Bookings:   

  • The Hirer must state the type of activity to take place and use the facility for that sole purpose 

  • The Hirer must use ONLY the area in the facility that has been booked 

  • Any changes to the booking such as times or content, must be communicated to and agreed by Heart Place Hospital management in writing/email before the Hire period takes place 

  • All hireage must be paid for at the time of booking. 

  • Set up and pack down must be included in the Hire period and HPH space must be vacated by the time stipulated in the agreement. 

  • HPH rooms and space must be cleared of all rubbish and left in the same condition as when entered.  Cleaning supplies can be found on premises. 

 

Cancellations: 

  • Should the Hirer want to terminate the Agreement, Heart Place Hospital will refund as follows: 

  • More than 10 working days prior to the Event - full refund 

  • 5-10 working days prior to the Event - 50% refund 

  • Less than 5 working days prior to the event - NO refund 

  • Heart Place Hospital may terminate booking(s) if it considers the Event will, or might, contravene any statue, order, regulation, bylaw, rule or law or requirements of a public or local authority 

  • Heart Place Hospital reserves the right to cancel a booking(s) where circumstances so warrant.  These may include, but are not limited to, emergency situations and adverse weather conditions 

  • Heart Place Hospital can, at its sole discretion, terminate the Agreement. Any monies paid in advance for the booking will be refunded 

Payments: 

  • A bond/deposit is required at time of booking and is refundable after satisfactory inspection upon hire completion. There is also an option to gift the bond to HPH as a donation with a receipt provided. 

  • The room hire invoice is to be paid at least two weeks in advance of the event:
